You may sign up CHC membership by dropping off a note with your name (English, Chinese optional), phone number, email address, home address, and Sun City ID number along with a check payable to CHC.  Drop it off to the CHC mailbox outside of Fitness Center at Anthem Center, or submit it to any CHC board members.​
